Course Schedules and Enrollment Forms

After reviewing the course schedule and determining which course or courses you would like to take during the next term, please complete the enrollment form (attached below). Although the enrollment form is in a PDF format, you will find it easy to type your information on the form before printing it out. Once completed, please fax or send the form(s) to the Office of the Registrar. If you have additional questions, please feel free to contact the Office of the Registrar Monday through Thursday between the hours of 8:30 a.m. and 6:30 p.m. or on Friday between 8 a.m. and 5 p.m. at 602-371-1188.
