The 2021-2022 tuition and fees schedule is effective from 7/1/2021 through 06/30/2022.

Student Health Insurance (estimate) $750 $1,500

NOTE:  All students at OUAZ are required to carry a primary health insurance policy, it is not optional. This policy must incorporate all ACA requirements and must provide all major services within a 50-mile radius of the 85374 zip code (Surprise, AZ). To determine if your policy meets these requirements, please send a copy of the front and back of the student’s insurance card to your Enrollment Advisor. There is also an Insurance Waiver Form that will need to be completed as part of the Admissions Paperwork prior to Orientation. Once that information is on file, it can be determined if your health insurance provides adequate coverage and the charge will be removed. Please note: we’ve found it difficult for students with Medi-Cal, Nevada Medicaid, and/or Medi-Share to get approved for medical services in the Phoenix metropolitan area. **All International students will be required to carry the OUAZ primary health insurance policy.
